Affidavit Forms

Tournament Verification Checklist
- Each league should fill out their league ID, league name, division, players names (as listed on birth certificate).
- DO NOT FILL OUT ANY OTHER SECTION. District will complete the remainder of the form during the Affidavit Signing meeting.

Reminders:
- All forms must be dated after May 15th, 2022.
- Player name must be their full name as listed on their birth certificate for all the forms above.
- Birth certificate must be the original copy with the raised seal. No hospital record form or certificate will be accepted
- After the Affidavit is signed off by District, copies of birth certificates are acceptable to keep in team binders. Official birth certificates can be given back to parents.

Waivers

Little League Residency Waiver Request
- This waiver releases the player to play in the Little League out side of their boundary.
- The player IS NOT eligible for selection to a Tournament (All Star) team for such season.
- Prior to sending to District, the request form should be notarized.

II(a) and II(d) Waiver
- Regulation II rule allows a player who previously qualified under residency for a league and played in the league to remain in the league after having moved to a new residence or school.
- Player must play continuously (without an any seasons off or with another league).
- As long as the player lived within the boundary and played with the league, they may continue service with the league if the player’s address or school location changed, the league’s boundary has changed, or the player is a sibling of a player who previously qualified for II(d).

IV(h) Waiver
- If a person has previously resided within the league’s boundaries for two or more years while serving that league as a dedicated manager, coach, or member of the local league’s Board of Directors for two or more years, their child(ren) are eligible to try out and be selected by teams in that league provided such service to the league from which the person has moved has continued and subject to written agreement from the league within whose boundaries they currently reside.

Reminders:
- All waivers need to be sent to the District Administrator.
- II(d) and IV(h) must be approved by District Administration.
- Waivers must be approved prior to the Affidavit book signing meeting.
